Who we are
Namita Azad is an executive healthcare leader with expertise in health system delivery development including integrated clinical service programs and value-based care models. With 20+ years in the industry, she has led complex redesign efforts of operational and clinical pathways that improve key equity, quality and safety metrics in large academic medical centers across the United States including Columbia University Medical Center, Montefiore Health System and University of Wisconsin Health. She is passionate about leading organization transformation initiatives at the grassroot level by leveraging change management models and believes in optimizing human capital in order to provide high quality, accessible care to diverse patient communities.
Emily Hart is an accomplished healthcare executive with expertise in performance improvement, change management, program development and program evaluation. Beginning her professional career 16 years ago as a direct care Licensed Social Worker, Emily has built upon her clinical proficiency to become a leader in identifying and implementing improvement opportunities in complex multi-modal systems that are rooted in delivering improved outcomes in quality and safety, patient experience, regulatory compliance, and financial performance. Managing large teams focused on driving transformation, Emily has held leadership positions across the east coast at Montefiore Medical Center, Yale New Haven Health System and Silver Hill Hospital. Emily firmly believes in the practice of engaging all levels of an organization to solve for root cause challenges as a mechanism to build intrinsic motivation for embedding and maintaining change.

Managed the Value Based Purchasing Quality Improvement Program portfolio, evaluated at $176 million, for the Montefiore Health System and lead systematic change to make impact in areas of Hospital Acquired Infections, HR Readmissions & ED throughput
Designed and deployed Columbia University Irving Medical Center’s 1st telehealth model supporting care delivery to over 500,000 patients during the COVID-19 pandemic
Led the clinical operational transformation of a 26 sub-speciality health system, composted of 15000 clinicians, through a large scale technology transition
Designed and executed a Quality Improvement Learning Collaborative Program, modeled on the IHI breakthrough series and incorporating lean & six-sigma concepts, across 16 hospital systems impacting over 80 frontline teams
